Natural dermal fillers: how a natural result happens
A natural filler result is not a product you can ask for by name. It comes from four decisions made in the right order: which gel, how deep, how much at once, and whether to stop. We have written before about how much filler is too much. This post is about how the good result happens in the first place.
Product choice: one gel does not do every job
All the fillers we use are hyaluronic acid, a sugar your skin already contains, cross-linked into a gel so it holds its shape for a year or more. Within that family the gels differ a lot. Concentrations in the ranges we stock run from 15 mg/ml for fine surface lines to 25 mg/ml for rebuilding a cheek or jawline. A firm gel made to hold up the mid-face will look like a ridge if it is put in a lip line. A soft gel made for fine lines will vanish under the weight of a cheek and do nothing.
Natural results start with matching the gel to the tissue. For the cheeks, chin and jawline we use the firmer, higher-concentration products, placed deep. For folds and lines we use the mid-range gels. For the finest lines we use the softest. We stock Revolax, Teosyal and Juvederm, which lets us pick by job rather than by brand loyalty, and we will tell you which one we are opening and why.
Placement: depth is what the mirror sees
The same millilitre looks different at different depths. The manufacturers' own instructions for the volumising gels say deep placement only: in the fat under the skin or directly on the bone, never in the skin itself. Put deep on the bone, a firm gel lifts the tissue above it and the surface stays smooth. Put too close to the surface, the same gel shows as a bluish tint, a lump you can feel, or a shelf that moves when you smile. The bluish tint, called the Tyndall effect, can last months or years if left.
So we place structural filler on the bone in small amounts, we use a blunt cannula for the jawline and cheeks, and we never use a cannula thinner than 25 gauge, because thinner ones have been shown to enter an artery as easily as a needle. We look at your face moving, not only at rest, before and after each small amount. The aim is that nothing is visible except the change in shape.
Staged treatment: less now, review, then decide
The quickest way to an unnatural face is to do everything in one visit. The instructions for one of the jawline gels we use set a ceiling of 2 ml per area per session and a minimum of two weeks between injections. In the cheek study behind another product, people received a median of 6.6 ml in total, but across an initial visit and a touch-up about a month later, not all at once.
Our working rule for a first visit is no more than 4 ml across all areas, and often much less. Swelling makes every fresh filler look fuller than it will, so judging the final result on the day is a mistake. We treat the first visit as the foundation, and we build only if the settled face asks for it.
The review at two weeks: where the decision is made
In the cheek study, tenderness affected 92 in 100 people, swelling 86 in 100 and bruising 78 in 100. The moderate or severe reactions lasted a median of 2 days and settled completely in a median of 6 days. The manufacturers say most reactions are gone within a week. That is why we book a review at 2 to 4 weeks and not before: at two weeks you are looking at the real result.
At the review we photograph you in the same light as before, check for lumps and symmetry, and talk about whether to add anything. Often the answer is no. A natural result is usually one that the person's friends cannot name. Most people see the effect for about 12 to 18 months. In the cheek study, 85 in 100 still had a visible improvement at 12 months and 67 in 100 at 24 months.
Saying no: the part most clinics skip
We decline filler more often than people expect. We do not inject the nose, the area between the brows or the forehead. A 2019 review found 93 published cases of vision loss after facial filler, nearly half from the nose, and in 72 in 100 of those cases sight did not improve. No cosmetic change is worth that odds table. We do not treat anyone under 18, which has been the law in England since October 2021. We do not add filler to a face where we cannot account for what is already there, and we will not use filler to chase sagging skin that needs a different approach.
If a result is wrong, filler can usually be dissolved with a prescription-only medicine, used off licence for this purpose, and we hold it in clinic. It is an undo button with its own risks, not a reason to be casual on the way in.
Who it suits, and who it does not
Filler suits volume that has gone from the cheeks, chin, jawline or temples, and folds that have deepened with it. It does not suit loose skin without volume loss, where HIFU may be the better first step, or dull, thin, crepey skin, where a skin booster is more honest than a structural gel. It is not for anyone under 18, anyone pregnant or breastfeeding, anyone with an active skin infection in the area, or anyone whose expectations do not match what a gel can do.
Risks and recovery
Common and short: swelling peaking at about 48 hours and settling by day five, bruising for up to two weeks, tenderness and small lumps you can feel for one to two weeks. Less common: lumps that stay or appear weeks later, infection, a bluish tint, or the gel shifting. Rare but serious: a blocked blood vessel, estimated at about 0.01 to 0.05 in 100 treatments and under-reported, which can damage the skin, and very rarely loss of sight. We screen carefully, inject slowly in small amounts, keep you in clinic for 30 minutes afterwards and give you a number for the same day. Results vary and cannot be guaranteed.
FAQ
Which filler brand gives the most natural results?
None by itself. The natural look comes from the right gel for the area, placed at the right depth, in a small enough amount. The same brand can look natural in one face and heavy in another.
How much filler do I need for a natural look?
Usually less than you expect. We start conservatively, review at two weeks and add only if the settled result needs it.
Will people be able to tell I have had filler?
If it is placed well, they should notice you look rested rather than notice filler. Swelling in the first few days is the only stage where it may be obvious.
Can natural filler results be reversed?
Hyaluronic acid filler can be dissolved with a prescription-only medicine, used off licence, and your body also breaks the gel down over 12 to 18 months.
How much does it cost?
Per 1 ml: Revolax £130, Teosyal £240, Juvederm £280. We tell you the volume and the total before anything is opened.
